Recommendation Request Instructions

To request SCHOOL FORMS/COUNSELOR FORMS/COUNSELOR RECOMMENDATIONS:

 

  1. Print out or detach the counselor form(s) from the application(s).
  2. Attach the form(s) for each college to a stamped, addressed envelope for that college. Include one copy of your current activities list.
  3. Place all materials in a large envelope clearly labeled "Counselor Form Request" with your name and the due date.
  4. Give this packet to Mrs. Turner no later than two weeks before it is due to the college.

 

To request TEACHER RECOMMENDATIONS:

 

  1. Print out or detach the teacher form(s) from the application(s).
  2. Attach the form(s) for each college to a stamped, addressed envelope for that college. Include one copy of your current activities list.
  3. Place all materials in a large envelope clearly labeled "Teacher Recommendation Request" with your name and the due date.
  4. Give this packet to your teacher no later than two weeks before it is due to the college.  
  5. Send a thank you note to your teacher.
